HELP Installing Salesforce Lightning - Softphone not displaying correctly

I am installing RingCentral into Salesforce Lightning. I was able to finish everything except the very first step regarding the Visualforce Pages. I could not find the OpenCTIIndex999 in the manual.

Please help :)
